We take accuracy seriously. The data we publish reproduces public records published by the Centers for Medicare & Medicaid Services (CMS). This page explains how to raise a concern about anything we publish, and what we will do in response.

If a figure we publish looks wrong

There are two different situations, and they are handled differently.

The CMS record itself is wrong. Our pages mirror the CMS data. If the underlying record at CMS is incorrect, the correction needs to happen at the source, because that is what feeds our page and every other site that uses this data. Facilities can pursue corrections and dispute findings through the CMS process and their state survey agency. Once CMS updates the record, our page updates on the next monthly refresh. If you tell us a CMS correction is in progress, we can note it in the meantime.

Our page does not match the current CMS record. If we have made a reproduction error, so that a value on our page differs from what CMS currently publishes, that is our error and we will correct it. Please send us the facility name, the CMS Certification Number, the specific field in question, and what the current CMS record shows.

How to reach us

Send correction and takedown requests to corrections@nursinghomerecord.com. To help us act quickly, please include:

  • The facility name and its CMS Certification Number (CCN).
  • The exact page or figure at issue.
  • What you believe is wrong, and, if you have it, the current CMS record or a link to it.
  • Whether a correction is already underway at CMS or the state survey agency.
  • Your name and how we can reach you if we have questions.

Our response commitment

We aim to acknowledge correction requests within five business days. Where the issue is a reproduction error on our side, we will fix the page promptly once we can confirm the current CMS record. Where the issue is with the CMS record itself, we will explain the source process and, on request, note that a correction is pending. We keep a record of requests and what we did in response.
